Who Qualifies:

To qualify for financial assistance through the Kelly Anne Dolan Memorial Fund’s Financial Assistance Program, a child must meet the following criteria:

  • all referrals must be made by a social worker, nurse, case worker or other healthcare provider currently familiar with the child and his or her illness and family situation;
  • the child must have a diagnosed serious, chronic or critical illness, disability or condition currently requiring medical attention, though he or she does not need to be hospitalized at the time of the referral;
  • further, the child must be medically involved enough that he or she is currently being followed by a healthcare provider who is informed about the child’s condition;
  • the child must be a resident of Pennsylvania, New Jersey or Delaware, though may be receiving treatment in another state;
  • the child must be dependent on parents or guardians;
  • the Fund helps both low income and middle income families and is committed by its Mission to helping families who are experiencing financial challenges as a direct result of the level of care required by their child; for example: one or both parents or guardians have lost a job as a result of time off from work to care for their sick child, or have had to cut back on work in order to care for their medically involved child;
  • young adult children who have been seriously ill since childhood and continue to be medically involved will be considered for financial assistance on an individual basis; typically, other forms of assistance are available to help adult children which the Fund will try to help the referring healthcare provider identify;
  • the Fund’s Data Sheet and Cover Letter must be filled out completely; all information requested is strictly confidential and required in order for us to process the application as well as to generate anonymous statistical reports for our funders and other constituents. If the paperwork is not properly completed, the request for assistance will be delayed or, possibly, denied.
